Accessing Medical Records
The Health Informatics and Information Management Department maintains medical records related to care received at all Vista Health System facilities. The release of confidential medical records is governed by federal regulations established under the Health Insurance Portability and Accountability Act (HIPAA).
Copies of medical records may be released upon receipt of a completed Authorization to Disclose Protected Health Information form. This form must be completed by the patient (18 years of age or older) or the patient’s parent or legal guardian, as applicable.
Only the patient, parent or legal guardian, or the patient’s legally authorized health care representative may sign the form to authorize the release of medical records. If you are requesting records on behalf of a patient or as the patient’s legal representative, please provide a copy of the applicable legal documentation, such as an Advance Directive, Durable Power of Attorney for Health Care, or Conservatorship documentation.
Vista Health System has partnered with Sharecare to fulfill medical records requests.
Vista Health System is committed to protecting your medical information. For information about your rights and our obligations regarding the use and disclosure of your medical information, please review our Notice of Privacy Practices.
